Medical Hardship Enrollment Reinstatement
A request to reinstate a course or courses is for a student who has a Medical Hardship hold in place. Once this form is completed NDCDE will contact the person requesting the reinstatement as well as the person who purchased the course enrollment to verify the reinstatement request.
